Open your Silvertree app.

Click on 'manage our Care Team'

Click on 'modify the team'

Scroll down to the bottom of the screen and click on 'Reassign Primary Care Partner' (you should see this option below 'Invite a new Care Team member')

Check the box next to the Care Partner you would like assigned as your Primary Care Partner.

Click 'reassign'

Now you're all set! You can use the back arrow in the top left corner of the screen to navigate back to your main app dashboard.
